2. Special permit from the Department of Public Works and Highways (DPWH) if the gross vehicle weight of the motor vehicle exceeds the limitation imposed by LOI No. 112
3. Clearance from Philippine National Police -Traffic Management Group (PNP-TMG)
4. Appropriate Insurance Certificate of Cover
5. Motor Vehicle Inspection Report (MVIR) [Download this form here]
6. Copy of Dealer's Report duly authenticated by the Assistant Secretary with Official Receipt (OR)
7. Taxpayer's Identification Number (TIN)
Rebuilt Motor Vehicles (Assembled/Backyard) By Non-Accredited Manufacturers (Private Motor Vehicles Only)
Rebuilt/Assembled Motor Vehicles Out Of Imported Components (Chop-Chop)
1. Commercial Invoice of the engine, chassis and body
2. Confirmation of the Certificate of Payment of the engine, chassis and body issued by the Bureau of Customs (BOC)
3. Copy and Confirmation of Clearance Certificate of Motor Vehicle (CCMV) by the Bureau of Internal Revenue (BIR)
4. Affidavit of Assembly / Rebuilt executed by the Owner and Mechanic, stating among others, the date of completion of the motor vehicle
5. Clearance from Philippine National Police -Traffic Management Group (PNP-TMG)
6. Appropriate Insurance Certificate of Cover
7. Actual inspection of MV with duly accomplished MVIR [Download this form here]
8. Confirmation of Dealer's Report from Registration Section / Regional Office
9. Taxpayer's Identification Number (TIN)
10. Certificate of Emission Compliance from PETC Rebuilt Locally Assembled (Backyard)
1. Commercial Invoice of the engine, chassis and body
2. Affidavit of rebuilt executed by the Owner and the Mechanic, stating, among others, the date of completion of the Motor Vehicle
3. Clearance from Philippine National Police -Traffic Management Group (PNP-TMG)
4. Appropriate Insurance Certificate of Cover

1. Copy of Certificate of Registration (CR) and latest Official Receipt (OR) of payment
2. Affidavit of Rebuilt executed by the Owner and Mechanic
3. Deed of Sale, if engine, chassis and body are acquired from different persons or entities
4. Clearance from Philippine National Police -Traffic Management Group (PNP-TMG)
5. Appropriate Insurance Certificate of Cover
6. Motor Vehicle Inspection Report (MVIR) [Download this form here]
7. Confirmation of CR/OR (CIR-91-137) if issued by other LTO Agency
8. Taxpayer's Identification Number (TIN)
